Social Prescribing enables health and social care professionals eg GP’s, Care workers and Dementia navigators to support patients to connect them with local community groups and agencies to aide their physical and mental well being.
Think active is working to increase the rate of social prescribing to community-based sport and physical activity groups, clubs and facilities.

Provide training for social prescribing volunteers, buddies and navigators so they are able to advocate and positively support patients to access community sport/physical activity opportunities.
Build awareness, confidence and trust between the social prescribers and community sport and physical activity provision so the health referral workforce feels confident that this provision is appropriate, suitable and able to meet their patients’ needs and that the community provision is confident to welcome patients.
Support community sports and physical activity clubs, groups and facilities to ensure a customer centred, safe and welcoming experience is being provided which meets the needs of the person referred.
